Anki for Step 1: The Strategy That Actually Works

By a Resident Physician  ·  Updated 2025  ·  10 min read

Anki is the most powerful study tool available for Step 1 — and also the one most commonly misused. The difference between Anki working for you and consuming your life is strategy.

Why Anki Works (The Science)

Anki uses spaced repetition — a system that resurfaces information at precisely the moment before you'd forget it. Research on spaced repetition consistently shows 80–90% long-term retention compared to 20–30% for re-reading. For Step 1, where you need to retain 10,000+ facts over 8 weeks, there is no better tool.

The AnKing Deck: What It Is and How to Set It Up

The AnKing deck (originally Zanki, now maintained by the AnKing community) is a 25,000+ card deck that integrates First Aid, Pathoma, Sketchy, and B&B tags. Every card is tagged by source, organ system, and topic. This tagging is what makes it manageable.

Setup steps:

  1. Download Anki (free) and install the AnKing deck from ankingmed.com
  2. Suspend the entire deck immediately — you start with zero active cards
  3. Unsuspend by tag, organ system by organ system, as you study
  4. Only unsuspend cards for content you've already studied in Pathoma/Sketchy that week

Daily Card Limits That Don't Destroy You

Weeks 1–2: 50 new cards per day

This generates roughly 100–150 reviews per day by Week 2 — manageable at 30–40 minutes.

Weeks 3–5: 75 new cards per day

Reviews build to 200–250 per day. This is your sustainable high-output window.

Weeks 6–8: 30 new cards per day from UWorld, 0 from deck

Add only cards from UWorld explanations for your personal weak areas. Your review burden is now 300+ per day — protect 60–90 minutes for reviews daily.

How to Actually Do Reviews (Not Passively)

The most common Anki failure mode is passive reviewing — clicking through cards quickly, pressing "Good" even when uncertain. This feels productive but builds false confidence.

The correct approach: For each card, verbalize or write the answer before flipping. If you can't, press "Again" even if you sort-of remember. The algorithm needs accurate signal. A card rated "Good" when you were uncertain will be seen too infrequently in the future.

When to Suspend Cards

Anki on Mobile: Making Dead Time Count

Reviews — not new cards — can be done on AnkiMobile during commutes, meals, and breaks. This is legitimate, effective use of the app. Never add new cards on mobile — adding new cards requires your full cognitive engagement. Reviews, by design, are lower intensity.

The burnout warning: Students who do 400+ new cards per day in Week 1 inevitably hit a wall in Week 4–5 when their review burden becomes 600+ cards per day and they can't keep up. Sustainable daily minimums beat aggressive early starts every time. Anki debt compounds exactly like financial debt.
