Pathoma for Step 1: How to Use It and What to Focus On

Pathoma is not just a resource — it's the framework around which the rest of your Step 1 preparation should be organized. Here's how to use it correctly and what to focus on in each chapter.

Why Pathoma Is Different From Other Resources

Dr. Sattar explains pathology the way a physician thinks about disease — starting from a normal physiological state and walking through the mechanism by which it breaks down. This mechanistic approach is exactly what Step 1 tests. Most other resources teach what happens; Pathoma teaches why.

How to Watch Pathoma Videos

First viewing: comprehension

Watch at 1x speed with the companion book open. Do not take notes — just follow along and let the mechanism sink in. Sattar's explanations are dense; trying to transcribe them prevents you from actually understanding them.

Second viewing: active recall

Pause after each major concept and close your eyes. Can you explain the mechanism back in your own words? If not, rewind. This is where the learning actually happens.

Chapter-by-Chapter High-Yield Focus

Chapter 1: Cell Injury

Foundation for everything. Understand the types of necrosis (coagulative, liquefactive, caseous, fat, gangrenous, fibrinoid) and the examples for each. Apoptosis vs. necrosis is tested frequently. Free radical injury mechanism is high yield.

Chapters 2–3: Inflammation and Healing

Know the mediators of inflammation and their functions. The complement system (C3a, C5a, MAC). Granuloma formation and the diseases that cause it (TB, sarcoidosis, Crohn's, foreign body). Wound healing steps and complications.

Chapter 5: Cardiovascular

Highest-yield Pathoma chapter. Deep dive on atherosclerosis, MI (zones of infarction, complications by day), cardiomyopathies (dilated vs. hypertrophic vs. restrictive), endocarditis (organisms by risk factor), and valvular disease.

Chapter 6: Red Blood Cell Disorders

Anemia classification (microcytic, normocytic, macrocytic), hemolytic anemias, sickle cell complications, thalassemias. Know the laboratory findings that distinguish iron deficiency from anemia of chronic disease.

Chapter 14: Kidney

Nephritic vs. nephrotic syndrome is one of the most tested distinctions in all of Step 1. Know the specific diseases associated with each pattern and their light microscopy, electron microscopy, and immunofluorescence findings.

Integrating Pathoma With First Aid and Anki

After each Pathoma section, open First Aid to the corresponding organ system and annotate it with the mechanisms Sattar explained. These annotations transform First Aid from a fact sheet into a mechanistic reference. Then add Anki cards for the specific details (names, percentages, specific findings) that Pathoma explained but that you'd need to memorize specifically for the exam.

The most common Pathoma mistake: Watching the videos passively while highlighting the book. Pathoma's value is in the explanations — if you're not actively processing what Sattar is saying, you're getting 20% of the resource's value. Pause. Explain it back. Then move on.
