How opportunistic infections are tested on Step 1
Vignettes give an immunosuppressed host and a syndrome and ask for the organism, the stain, the prophylaxis threshold, or the drug.
Ask which arm of immunity is missing. Each arm defends against a characteristic set of organisms, so the defect narrows the differential before any test result.
Match the defect to the organism
Primary and acquired immune defects produce overlapping patterns.
The HIV CD4 ladder
As CD4 counts fall, infections appear in a predictable order. Antiretroviral therapy reverses the ladder as counts recover.
| Organism | Syndrome | Diagnosis | Treatment |
|---|---|---|---|
| Pneumocystis jirovecii | Dry cough, hypoxia, bilateral ground-glass opacities | Silver stain of BAL; ↑ LDH, ↑ β-D-glucan | TMP-SMX; add steroids if PaO₂ < 70 mm Hg |
| Toxoplasma gondii | Multiple ring-enhancing brain lesions | Positive IgG; response to therapy | Sulfadiazine + pyrimethamine + leucovorin |
| Cryptococcus neoformans | Subacute meningitis, high opening pressure | India ink; capsular antigen | Amphotericin B + flucytosine, then fluconazole |
| CMV | Retinitis (cottage cheese and ketchup), colitis, esophagitis | Funduscopy; owl's eye inclusions | Ganciclovir or valganciclovir |
| Mycobacterium avium complex | Fever, weight loss, diarrhea, ↑ alkaline phosphatase | Blood culture | Clarithromycin or azithromycin + ethambutol |
| JC virus | PML: nonenhancing white-matter lesions | MRI; JC PCR in CSF | ART; no specific antiviral |
Starting ART can unmask or worsen an infection — immune reconstitution inflammatory syndrome (IRIS).
Other immunocompromised hosts
Chemotherapy, transplant, and asplenia produce their own patterns.
A 38-year-old with untreated HIV and a CD4 count of 80/mm³ has 2 weeks of headache and fever. Neck stiffness is mild. Opening pressure on lumbar puncture is 32 cm H₂O, and CSF shows encapsulated budding yeasts. What is the next step beyond antifungals?
- Encapsulated budding yeasts in CSF at CD4 < 100 mean Cryptococcus neoformans.
- Induction therapy is amphotericin B plus flucytosine.
- Raised intracranial pressure is a major cause of death in cryptococcal meningitis.
- Serial therapeutic lumbar punctures relieve the pressure; ART is delayed a few weeks to reduce IRIS.
Answer. Repeat therapeutic lumbar punctures to lower intracranial pressure, with ART started about 4–6 weeks after antifungal induction.
Prophylaxis in HIV
Primary prophylaxis prevents first episodes; it can stop once ART raises the CD4 count above the threshold for about 3 months.
| CD4 threshold | Organism | Prophylaxis |
|---|---|---|
| < 200/mm³ | Pneumocystis jirovecii | TMP-SMX (alternatives: dapsone, atovaquone) |
| < 100/mm³ and Toxoplasma IgG positive | Toxoplasma gondii | TMP-SMX |
| < 50/mm³ (not starting ART promptly) | Mycobacterium avium complex | Azithromycin — no longer routine if ART is started |
| Any CD4, positive TB screen | Latent tuberculosis | Isoniazid (with pyridoxine) or a rifamycin-based regimen |
Dapsone can cause hemolysis in G6PD deficiency and methemoglobinemia.
