Gross Motor Milestones
Gross motor follows a cephalocaudal progression. 2 months: holds head up when prone (no head lag). 4 months: rolls front to back. 6 months: sits with support; rolls back to front. 9 months: pulls to stand; crawls. 12 months: cruises along furniture; first steps (range 9–15 months). 15 months: walks independently. 18 months: runs (stiff, wide-based). 2 years: walks up stairs with both feet on each step; kicks a ball. 3 years: climbs stairs alternating feet; jumps off step; rides tricycle. 4 years: hops on one foot; skips. 5 years: skips smoothly; rides bicycle with training wheels. Red flags: not walking by 18 months; not running by 2 years; toe-walking with stiffness (consider spastic cerebral palsy, Duchenne muscular dystrophy). Regression (loss of milestones) at any age = immediate evaluation.
Language and Fine Motor Milestones
Language: 2 months: coos. 6 months: babbles (consonant-vowel combinations — ma, ba). 9 months: mama/dada non-specifically; stranger anxiety begins. 12 months: mama/dada specifically; 1–2 words besides mama/dada; waves bye-bye. 15 months: 3–6 words. 18 months: 10–25 words; points to show. 2 years: 50+ words; 2-word phrases ('want milk'). 3 years: 3-word sentences; 75% intelligible to strangers. 4 years: 4-word sentences; asks 'why'; 100% intelligible. 5 years: tells stories; knows full name, age, address. Red flag: no babbling by 12 months, no words by 16 months, no 2-word phrases by 24 months, any loss of language. Fine motor: 3 months: open hands. 6 months: transfers objects hand to hand. 9 months: pincer grasp (thumb + index). 12 months: pincer grasp refined; bangs 2 cubes. 15 months: puts 3 cubes in cup; scribbles. 18 months: tower of 2–3 cubes; feeds self with spoon. 2 years: tower of 6 cubes; turns pages; removes clothing. 3 years: tower of 9; copies a circle; dresses self. 4 years: copies a cross (+). 5 years: copies a triangle; ties shoes.
Social and Autism Screening
Social milestones: 2 months: social smile. 3 months: recognizes parent face. 6 months: stranger anxiety beginning; laughs. 9 months: object permanence (searches for hidden toy). 12 months: stranger anxiety peak; separation anxiety; shows joint attention (follows finger point). 18 months: parallel play (plays alongside, not with); symbolic play begins. 2 years: associative play. 3 years: cooperative play. 4 years: has preferred friends. Autism spectrum disorder (ASD) red flags: no social smile by 2 months, no babbling or pointing by 12 months, no words by 16 months, no 2-word phrases by 24 months, loss of any skill at any age. Screening: M-CHAT-R/F at 18 and 24 months (Modified Checklist for Autism in Toddlers). Key features: deficits in social communication + restricted/repetitive behaviors. Diagnosis: age ≥2 by multidisciplinary team. Early intensive behavioral therapy improves outcomes.
